Default N. sanguinea problem...

My N. sanguinea doing not well lately. Ok let start from beginning. Bought this plant last year with some kind of brown sand as a medium and place under full sun. About 1/2 month ago the leaf initially produce yellow spots so i move to under 50% shade with other neps. Then it it got worst and more yellow spot produce and the tip is like showing bulgy/ bumpy circles on it. Worry, i change to new medium (sphag + shredded bark + perlite in equal). Few days later the tendril start to blacken. Below is a pics taken 1 week + ago. Now, what on earth did i do wrong this time??
